So Cal Quality Review

Medical record review for life care planners.

A life care plan is only as strong as the record behind it. We make that record complete, organized, and clearly summarized, so you can spend your time on the plan instead of the paperwork.

What we do

  • Record organization and indexing

Records from every provider are sorted, deduplicated, paginated, and indexed into one organized set with a table of contents, so any page can be found and cited in seconds.

  • Medical chronologies

A date-ordered account of treatment: visits, diagnoses, procedures, imaging, medications, and provider recommendations, each entry cited to the source page.

  • Narrative summaries

A plain-language summary of the injury or condition, the course of care, and current status, written to drop into the medical history section of a life care plan.

  • Gap and inconsistency review

We flag missing records, unexplained gaps in treatment, and conflicts between providers so they can be resolved before the plan is finalized.

  • Future care references

Where the record documents provider recommendations for future treatment, equipment, or follow-up, we collect them in one cited list for the planner's consideration.

  • Supplemental and updated records

When new records arrive, we fold them into the existing chronology and index rather than starting over, and we note what changed.

Who we work with

  • Life care planners

Our core clients. We deliver the record work in the format you already use, with citations that hold up under review.

  • Legal nurse consultants and case managers

Chronologies and organized records for case review, cost projections, and care coordination.

  • Attorneys and their staff

Organized, indexed records and summaries for plaintiff and defense matters where a life care plan is in play.

How an engagement works

  1. Send the records

Share the file through a secure transfer link we provide, along with the planner's questions and the deadline.

  2. Scope and quote

We confirm page count, what you need (index, chronology, summary, or all three), and the turnaround, then quote a flat fee for the work.

  3. Review and build

The records are organized and indexed first, then the chronology and summary are written from the organized set so every entry has a citation.

  4. Deliver and follow up

You receive the deliverables in your preferred format. Questions, clarifications, and supplemental records are handled as part of the same engagement.

Confidentiality

Medical records are handled with the care that medical and legal matters require. Files move through secure, access-controlled transfer, work is done on protected systems, and records are retained only as long as the engagement and your instructions call for.

So Cal Quality Review is based in Southern California and works with planners nationwide.
